We are currently accepting new patients, and our hours of operation are:
Monday – Friday: 8:30 a.m. – 12:00 p.m.
1:30 p.m. – 5:00 p.m.
Of course, there is always a physician available to meet your emergency medical eye needs.
Your appointment may take up to 2 hours. If we decide to dilate your pupils, your near vision may be a bit blurry. You may wish to bring your sunglasses or use those we provide.Your refills are important to us. Please be sure to verify at the time of service any need for refills of your medications.
Please arrive 10-15 minutes prior to your appointment time in order to complete the required paperwork or download the forms from our website and bring them with you. You will also need to bring your photo ID and insurance card(s).
We participate with a wide variety of insurance plans. Please bring your insurance card(s) with you so we can verify that we have the correct information to bill your insurance company. You will be asked to pay for all non-covered services at the time of the visit. We do not participate with any vision plans.
Patients with co-pay requirements must pay at the time of their visit. We accept cash, Visa, MasterCard, American Express, debit cards and personal checks. If your personal check is returned, you will be charged $40. Patients needing refraction (assessment of visual acuity and measurement for glasses) will be charged $50.00, collected at the time of service. This includes Medicare patients.

If your insurance carrier requires you to have a referral, you must bring a copy with you. You may also have your PCP fax the referral to our office prior to your visit. Without the referral, you will be asked to reschedule your appointment or pay for the visit in full at the time.
Please bring your eyeglasses and/or contact lens information as well as a list of all medications that you are currently taking and the dosage.
Please call our office if you need to cancel or reschedule you appointment. We would appreciate 24-48 hours advance notice for any appointment changes or you may incur a charge.
If you wish to save time, please print and fill out these forms and bring them with you to your appointment.
